Conditions of Enrollment & Enrollment Agreement My child is in good health and has permission to engage in all prescribed camp activities. Should a medical emergency arise, and in the event that a parent cannot be notified, I hereby give permission to the doctor selected by Cabaret Institute for Kids to hospitalize and secure proper treatment for my child. I agree to reimburse Cabaret Institute for Kids for any costs it may incur for the medical treatment of my child. I consent to the use of any photographs or videos taken of this camper for advertising or promotion by Cabaret Institute for Kids. I understand that the application fee ($25) is non-refundable.  I also understand that tuition is non-refundable after July 21, 2008.
